If you have photos of breasts similar to those you would like to have, we encourage you to bring them in; however, it is important to realize that each person's chest shape, muscles, skin tone and breasts are different. Dr. Nezami will discuss all these factors with you to help you determine the right breast implants for you and whether you might want a breast lift instead of or in addition to your breast enhancement surgery.
Ultimately, only you can decide on the size of your breast implants for your breast augmentation. Each person has her own preference and taste. This has to be entirely your decision. Keep in mind that your anatomy and tissue may not allow placement of very large breast implants, and that larger breasts sag faster than smaller breasts, resulting in the need for a breast lift later, to maintain your desired result.

There are different options available for breast implants. Dr. Nezami will explain the differences between textured breast implants and smooth breast implants. You have the choice of saline implants or silicone implants. The shell of both silicone gel-filled implants and saline filled implants are made of silicone. Saline filled breast implant is filled with sterile salted water; silicone gel-filled breast implant is filled with silicone gel. The two main manufacturers of both saline and silicone implants are Allergan and Mentor.
Silicone gel filled implants have more natural feel. Although silicone breast implants cost more than saline filled implants, many women love the natural feel of the silicone implants and are willing to pay the higher price. Silicone Implants are filled by manufacturers and have thick cohesive gel consistency. They retain their shape and they are firmer than saline implants and feel more like breast tissue. The old silicone implants were filled with liquid honey consistency silicone. The saline implants are filled with sterile salted water by the plastic surgeon. The plastic surgeon can adjust the volume of the same saline implant. All saline implants have a minimum and a maximum fill volume.
The silicone implant volume is fixed and cannot be changed after your breast augmentation surgery. This is why it is so important that the right volume of silicone implants is selected. You will select the volume and size of the your breast implants, but to obtain the best result Dr. Nezami will select the appropriate shape and profile of the implant that fits the best for your anatomy.
There are different profiles of implants, low profile, moderate profile and high profile. If the implant is too wide it will go more to the side and under the armpit. An experienced plastic surgeon is more likely to be able to help you to make the right selection of your breast implants. In our Jacksonville Cosmetic Surgery Center we use both Silicone and Saline implants. Regardless of the type of implants used; silicone or saline, all of our patients are extremely pleased with their new look and natural appearance of their breasts after their breast augmentation surgery.
When silicone implants rupture the leakage is not obvious whereas a ruptured saline implant decreases in volume or flattens. The change in the size and the look of the breast is noticeable right away since the body easily absorbs the saline solution. The rippling through skin and breast are easier seen with saline breast implants than silicone breast implants. For patients who have little breast tissue to cover the implant, Dr. Nezami will advise them to use silicone breast implants for their breast augmentation.
Both silicone and saline implants shells can be smooth or textured surface. The textured surface implants were developed because of their possible resistance to capsular contracture, but there is a higher incidence of leakage. The shape of the implants can be round or tear drop. Dr. Nezami prefers to use the round smooth surfaced implants for breast augmentation because they look and feel more like natural breasts. Another advantage of round breast implants is that rotation of the implant is not noticeable whereas the rotation of a tear drop shaped breast implant is noticeable.
At Jacksonville Cosmetic Surgery Center we have all the sizes and styles of implants. Our patients can try them. They can touch and feel the differences between saline implants and silicone implants. This will enable them to make a better choice for their breast implant. We encourage them to come with their spouse, relative or friend to our Jacksonville Cosmetic Center if they feel they need to have their opinion to help with their selection.
Another decision that you and Dr. Nezami will make together is whether to place your breast implants above or below the muscles of your chest during your breast augmentation surgery. Dr. Nezami will explain to you how your personal anatomy must guide this choice. The clinical studies indicate that placing the breast implants below the pectoralis muscle decreases the capsular contracture. Also it is believed that placing the breast implants under the muscles will improve the imaging of the breast tissue during mammography. In the long run placement of breast implants under the muscles will result in a more natural look.
If the patient opts for saline implants, the ripples will be less visible if the implants are placed under the muscles.
The incision for breast implants can be just above the crease below the breasts (Inframammary) or around the lower half of the areola (Periareolar or Infra-areolar) or under armpit (Transaxillary) or through umbilicus (Transumbilical). Dr. Nezami prefers the inframammary incision since it is less noticeable and is a direct access to under muscle or breast tissue where the implant will be placed. He makes the incision as small as possible. In some cases or if the patient request, Dr. Nezami will use the infra-areolar incision. He does not recommend transaxillary or transumbilical incisions.
We advise our patients in Jacksonville, Jacksonville Beach and Ponte Vedra Beach to avoid taking certain medication such as aspirin, Ibuprofen, Vitamin E, and any other medication that can affect the blood clotting system for two weeks before and after surgery (If you are taking any blood thinner, do not stop until you get authorization from the doctor who prescribed it). This is required to prevent any bleeding complications. If you are a smoker, stop smoking at least one month before and after the surgery. Smoking interferes with blood flow to the skin and prevents the rapid healing and may cause delayed wound healing or even tissue loss. Dr. Nezami advises his patients, two weeks prior to their breast augmentation surgery, to not take any herbal and diet medications without his approval.
About one week prior to the scheduled surgery, the patient will have a pre-operation and further consultation visit with Dr. Nezami at our Jacksonville Cosmetic Surgery Center. During that visit the patient will again try different sizers and make the final selection of her implant size. Dr Nezami will review the medical history and any record and reexamine the patient and perform some measurement necessary for selection of the implant style and profile. Considering the volume selected by the patient, he will select the best profile and style implant for that patient. Dr. Nezami will again explain the procedure, expected result, possible complications and alternative treatment and will answer all the patient's questions in detail.
Dr. Nezami and his surgical assistant will go over the pre and postoperative instructions, what to do and expect on the surgery day. The patient will have her folder containing her prescriptions and her pre and postoperative instructions. We will prepare the patient for stress free and smooth operation. They will have some blood test done at Baptist Medical Center if needed.
Patient is asked to wash her body especially her chest, breast and under arm with Hibiclean starting 3 days prior to and on the day of the breast augmentation surgery. Patient is advised not to wear deodorant or lotion on the day of the surgery and wear a button down sweatshirt and comfortable slip on shoes. Any jewelry on pierced body part should be removed. Patient should be fasting at least 6 to 8 hours prior to her surgery and avoid drinking red wine or alcoholic beverage 2 days prior to her surgery.
We ask our patients in Jacksonville and Ponte Vedra Beach to arrange for someone to drive them home and drive them back to our Jacksonville Cosmetic Surgery Center for their first post – operative visit. The patient needs to have someone stay with them for at least the first 24 hours after surgery.
Depending on the placement of the breast implants and the patient's anatomy, breast augmentation usually takes 1.5 to 2.5 hours.

This is a frequent question asked by our patients in Jacksonville and Jacksonville Beach. Breast Augmentation stretches the tissues, and this can be painful. When the breast implants are placed under the muscles, the pain might be a little stronger. The pain is most felt the first two days, after that, the pain lessens everyday. In any case, the pain can be controlled through medications. We ask our patients in Jacksonville and Jacksonville Beach to have someone to drive them home and help them for the first two days after surgery.
Sometimes you may experience sudden sharp short pain, burning sensation or sensitivity of your breast or nipple several days or week after surgery. There is no need to be alarmed, this is sign of the return of the sensation in some nerves that were stretch and will subside.
To help your healing process after breast augmentation, avoid stretching the muscles around the breast implants.
For three weeks you should avoid raising your elbows higher than your armpit. You will have a pink scar for several weeks, which will start to fade gradually after several months. You can start to work after the first week as long as your work does not involve lifting of heavy objects. The stitches will be taken out within five days. After breast surgery because of swelling and tightness of soft tissue, your breasts may seem unnatural. This is only temporary; gradually your breasts will soften and they will have a natural feel. You can start exercising after three weeks. Keep in mind that the recovery period varies from patient to patient.
To prevent any possibility of blood clot, we ask the patient to walk several times a day following their breast enlargement surgery.

Doctor Nezami will remove the tapes on your second post-op visit after 8 to 10 days. The patient can take a shower 2 to 3 days after their breast implants surgery. You should avoid hot tubs or baths. Five days after your breast surgery we advise you to start massaging your breast as instructed by Dr. Nezami. Massaging the breast will relax the tissue and prevent capsular contracture.

Keep in mind that every person heals differently. In general, the scarring is minimal; however, even with the application of perfect suturing techniques, every person may scar differently. There are a number of products on the market that can help the tissue to heal better and faster, such as vitamin E, Mederma creams Cortisone creams, and silicone sheets.
If you are considering breast augmentation, you must be at least 18 years old, in good health, and not pregnant or nursing.
For silicone implant you must be 22 years old. If your breasts have lost volume and are drooping, to enhance your breast you may need breast augmentation and breast lift. There is no upper age limit for breast enlargement or enhancement.
The majority of women who had their breast implants years ago are happy with their decision and state that they would do it again. Breast enhancement can increase a woman's self-confidence and self body image.
Both manufacturers of breast implants, Allergan and Mentor have limited warranty for their breast implants. The warranties for their breast implants are better than any other manufactured product on the market. Their breast implants warranties cover both types of implants; saline implants and silicone implants. The warranties cover only deflation or rupture of the breast implants. They do not cover the removal or exchange of the breast implants for capsular contracture, size alterations, or removal of breast implant due to rippling. Allergan warranty will replace the ruptured silicone implants and pay up to a given maximum amount of $3,500.00 for surgical expenses. For saline implants this amount is limited to $1,200.00. For $100.00 Allergan offers a ConfidencePlus Premier Warranty that will cover the out of pocket expenses such as surgical fees, operating room charges and anesthesia expenses up to $2,400.00 in case the saline breast implants rupture. The payment of expenses as mentioned above is limited for ruptured implants within the first 10 years from the breast implant surgery, but the implant replacement at no charge is for life. Therefore you will never have to pay for the implant if it ruptures or leaks even if at that time you decide to go larger and want to replace both with the same type implant.
Mentor Advantage Limited Warranty also covers for life the cost of the breast implant even if you replace both when one rupture with any size of similar style. In case of ruptured silicone Memory Gel implants, Mentor's warranty pays up to $3500.00 financial assistance in addition to free implants. In case of saline breast implants, Mentor will pay $1200.00 for surgery expenses, operating room and anesthesia charges. For $100.00 the patient can purchase a warranty up to $2,400.00 for her saline breast implants.
Breast enlargement surgery like any other surgical procedure involves some degree of risk. Patient must be ready to acknowledge and accept certain risk anytime that she goes under surgery. They are some risks that are beyond the control of your plastic surgeon, no matter how expertly and carefully he performs the surgery.
There are other type of risks that can be prevented if the patient does her homework and research the plastic surgeon's credentials, experience, past complications and frequency of the surgeon's complications. If the surgeon rushes through the surgery there will be a greater chance of overlooking some details. Read other patients testimonials. Ask a lot of questions during your first breast enlargement consultation in order to obtain a better opinion of your plastic surgeon.
Some complications although very rare that might occur as a result of breast augmentation surgery, unrelated to how well your plastic surgeon performs the surgery are:
Capsular contracture or hardening of the breast due to tightening of the breast capsule (scar formed around the implant) can occur and cause deformity, hardening and even some pain. If capsular contracture occurs the tight scars around the implant need to be surgically cut to give more space to the implant. Massaging the breast after breast implant surgery can help and reduce the chance of hardening of the breasts. Infection is rare but possible. During the breast enhancement surgery, the patient is given intravenous antibiotics to reduce the risk of infection. If there is infection around the implant, the implant must be removed and left out for 3 months to allow the infection to resolve.
In rare cases abnormal bleeding can occur after the breast enhancement surgery. In that case the patient is returned to the operating room to remove the blood. If the patient pulls or pushes hard with her arm or raise the arm above the level of hershoulder, she can tear the fibers of the muscle and cause bleeding. In that case the patient is returned to the operating room to remove the collected blood. Sensation of the nipple can change due to stretching of the nerve giving sensation to the nipple, it may become sensitive for short time and then return to normal. It is also possible to lose the sensation as previously discussed. Although rare, the implant can deflate due to malfunction of the fill valve in saline implant or rupture and tear of saline or silicone implant. The damaged implant needs to be replaced.
In-patient with thin breast tissue, there is more chance of having rippling with saline implant than with silicone implant and more often with textured implants than smooth breast implant. In this case, the implants must be removed and replaced with silicone implants.

A. Patient can go back to work after 5-6 days only if her job does not require heavy lifting, pulling, pushing or raising the arms above shoulder level.

Q. Can I take shower after my breast augmentation Surgery?
A. You cannot take shower before 3 to 4 days following your breast implant surgery, but you may sponge bathe or use hand held shower to wash below your breasts. You may not take bath for 2 weeks or swim for 3 weeks.
Q. Should I stop taking my medications before my breast enlargement surgery?
A. Q. Can I stop wearing bra after having my breast enlargement?
A. We advise our Jacksonville Cosmetic Surgery Center patients to not wear bra for the first few weeks until authorized by Dr. Nezami. After that he recommends to wear a very good support bra to prevent the weight of the implants from stretching the skin.
Q. Is it possible to have stretch marks as a result of my breast enhancement?
A. Some women may have stretch marks after their breast augmentation if they choose a very large size of implants; also it depends on their skin type. If they previously had stretch marks, it is possible that they might end up with some extra stretch marks after their breast augmentation. Over time the stretch marks will fade.
Q. I had my breast implants recently, I am not happy with the result. How long should I
wait before having my second corrective surgery?
A. Although we do not have unhappy patient on whom we have done breast augmentation, but our advice is to wait 3 to 6 months before having the corrective surgery to allow all the tissue to heal.
Q. I am a smoker; how long before my breast enlargement should I stop smoking?
A. For breast augmentation you should stop smoking at least 2 weeks before and after the surgery. Smoking interferes with blood flow to the skin and tissue and prevents rapid healing and can cause anesthesia and breathing problems. But if you are having breast lift in addition to your breast augmentation then you have to stop smoking for a minimum of one month after your breast enhancement surgery. You will need a good blood flow to the skin and nipple for proper healing. Smoking may cause delayed wound healing or even tissue or nipple loss.
Q. Will my areola become wider after my breast enlargement surgery?
A. Your skin will have to stretch to allow placement of the implants, the areola is also part of the breast and the skin that has to stretch. Therefore depending on the elasticity of your tissue the areola can become wider especially if very large implant is used.
Q. How soon after breast enlargement can my husband can touch my breasts?
A. There is no time restriction for very gentle touch. You should wait 2 to 3 weeks for forceful touch.
Q. When can I have sex after my breast augmentation surgery?
A. Two weeks after your breast enlargement surgery you can have sex if you play a passive role, after three weeks no restriction
Q. When can I lift heavy object after my breast enlargement?
A. Three weeks after your breast augmentation surgery you can lift heavy object and there are no restrictions
Q. Can I breast feed after my breast enhancement surgery?
A. Breast enhancement surgery does not affect your future breast-feeding ability.
Q. When can I sunbathe after my breast enhancement surgery?
A. Two weeks after your breast augmentation surgery you can sunbathe as long as you cover the incision area and it is not too hot.
